APCA Provider Credentialing Process
- A CHC will fill out a Request for Provider Credentialing form and
the Practitioner will complete a Practitioner Application form - Once received, APCA will begin the credentialing filing process
- APCA will provide weekly updates on the Practitioner’s file to the CHC
- APCA tailors to the needs of each CHC we provide services for
- Simply send documentation and we will take care of the rest!
Primary and Secondary Verification
- Primary: Education, National Practitioner Data Bank, Licenses,
Board Certifications, Employment History, and Professional References - Secondary: Immunization status, Life Support Trainings,
DEA Certification, and Government IDs

Initial/New: $499 Re-Credential: $399
