APCA Provider Credentialing Process

  • A CHC will fill out a Request for Provider Credentialing form and
    the Practitioner will complete a Practitioner Application form 
  • Once received, APCA will begin the credentialing filing process 
  • APCA will provide weekly updates on the Practitioner’s file to the CHC 
  • APCA tailors to the needs of each CHC we provide services for
  • Simply send documentation and we will take care of the rest!

Primary and Secondary Verification

  • Primary: Education, National Practitioner Data Bank, Licenses,
    Board Certifications, Employment History, and Professional References
     
  • Secondary: Immunization status, Life Support Trainings,
    DEA Certification, and Government IDs
     

Rates and Fees

Initial/New: $499                    Re-Credential: $399
